What name should I put in the Payee Legal Name field? Are nicknames allowed?
Complete the payee name field with your full legal name (first, middle, last). The payee name should be the legal name that matches a driver license, passport or other legal forms (i.e. Social Security Card or Visa). Nicknames are not permitted.
Do you need my full address?
Yes, your full address is required, which must include street address, city, state, and zip code for U.S. addresses. For addresses outside of the U.S., ensure you include city/region/province and country, as well.
What do I need to provide to receive electronic payment?
If you request electronic payment, provide complete banking information, including:
- Routing number and account number – U.S. Bank Payments
- Swift Code and IBAN – Foreign Bank Payments
- Sort Code required for Irish/UK banks
- Transit Code required for Canadian banks
Will I receive an email when my expense report has been processed?
Yes, as long as you provided your banking information on the form, a remittance advice will be sent to the email address which was used for the form completion. If that is not the email you would like your remittance sent to, notify your contact at the university. If you did not provide banking information, a check will be mailed to the address provided on the form and no email notification will be sent.
Do you need to know if I am U.S. Citizen?
Yes, ensure you answer the question regarding U.S. citizenship.
How can I attach receipts?
As you complete the payee section of the questionnaire, the ‘NEX Payee Receipts’ page will populate. Select the ‘Drop Files Here Or Click To Upload’ button to attach required or requested receipts.
Is there a time limit to work on the form?
Yes, the form will time out after 30 minutes of inactivity and any progress will not be saved if you have not clicked the Next button for that section.
